Submitting the EHCNA Request
Before submitting the EHCNA Request, please ensure that you have
included all required information, and that all necessary documentation is
attached to the form. Once submitted to the Local Authority, you will be
unable to amend the form.
To submit the form, select the 'Submit form' button within the 'Submit' section
of the form:
Upon submission, you will be navigated to a thank you page. You will also
receive an email to confirm that your form has been received.
The form you have submitted will be viewable within 'My Submitted Forms'
for 30 days; we recommend that you save a version as a PDF for your own
records as soon as possible.
